Timely Progress Review Notices (TPR)

Criteria for receiving a particular letter is listed below each notice.
 
 

Initial 24-Month Review Period Notices

Initial 24-Month TPR - Provider Letter (Qp1): 
  • Ticket has been assigned and in use with the same provider for 24 Months, and
  • Beneficiary has worked less than 3 months at Substantial Gainful Activity (SGA=$830 for 2005).
Passed Initial 24-Month TPR - Provider Letter (Gp1):  
  • Ticket has been assigned and in use with the same provider for 24 Months, and
  • Beneficiary has been deemed to pass due to evidence of work and earnings at or above SGA for 3 months or more, or
  • The EN/SVR has submitted the Intial 24-month TPR (Qp1) with a favorable response, or
  • The EN/SVR did not return the 24-Month TPR form or submitted an incomplete response to the notice (Qp1).
Unfavorable Initial 24-Month TPR - Provider Letter (Fp2): 
  • Ticket has been assigned and in use with the same provider for 24 Months, and
  • The EN/SVR has submitted the Initial 24-month TPR (Qp1) indicating that the beneficiary is not meeting one or more of the criteria to be considered actively participating:
    • The beneficiary did not actively participate in his/her employment plan, and/or
    • The beneficiary’s employment plan did not include a goal to achieve 3 months of work with earnings at/above SGA in the next 12 months, and/or
    • The EN/SVR did not expect the beneficiary to achieve the goal of employment for 3 months of work with earnings at/above SGA in the next 12 months.
Unfavorable Initial 24-Month TPR - Beneficiary Letter - English Version(F2);   Spanish Version(F2S):  
  • Ticket has been assigned and in use with the same provider for 24 Months, and
  • The EN/SVR has submitted the Initial 24-month TPR (Qp1) indicating that the beneficiary is not meeting one or more of the criteria to be considered actively participating:
    • The beneficiary did not actively participate in his/her employment plan, and/or
    • The beneficiary’s employment plan did not include a goal to achieve 3 months of work with earnings at/above SGA in the next 12 months, and/or
    • The EN/SVR did not expect the beneficiary to achieve the goal of employment for 3 months of work with earnings at/above SGA in the next 12 months.

First 12-Month Review Period Notices

First 12-month TPR – Evidence - Provider Notice (Qp2a):  
  • Ticket has been assigned and in-use for 36 months
  • The 1st 12-month TPR is due in the same month that the notice is being mailed (i.e., Ticket was assigned 08/2003, 1st 12-month TPR due in 09/2006, and a Qp2 notice is mailed in 09/2006)
  • Social Security Administration’s system reflects at least 3 months of earnings at SGA during the 36 months that the ticket has been assigned and in-use
  • Provider only needs to respond to the one question, which assesses whether or not the beneficiary is expected to work with earnings at/above Substantial Gainful Activity for 6 of the next 12 months.
First 12-month TPR - Provider Notice (Qp2b):  
  • Ticket has been assigned and in-use for 36 months
  • The 1st 12-month TPR is due in the same month that the notice is being mailed (i.e., Ticket was assigned 08/2003, 1st 12-month TPR due in 09/2006, and a Qp2 notice is mailed in 08/2006)
  • SSA’s system reflects less than 3 months of earnings at/above SGA during the 36 months that the ticket has been assigned and in-use
  • EN/SVR must address whether or not the beneficiary has worked with earnings above SGA for 3 months, and, if so, supply evidence of earnings for these months
  • EN/SVR must also address whether or not the beneficiary is expected to work with earnings at/above SGA for 6 of the next 12 months.
First 12-month TPR - Evidence - Provider Notice (Qp2c):  
  • Ticket has been assigned and in-use for more than 36 months
  • The 1st 12-month TPR is past due (i.e., Ticket was assigned between 01/2002-07/2002, 24-month TPR was not sent and beneficiary was deemed to pass, and 1st 12-month TPR was due 1/2005-07/2005)-- (will only be mailed 8/2005-9/2005)
  • SSA’s system reflects at least 3 months of earnings at SGA during the first 36 months that the ticket was assigned and in-use
  • EN/SVR only needs to respond to the one question, which assesses whether or not the beneficiary is expected to work with earnings at/above SGA for 6 of 12 months, beginning with the month the 2nd 12-month TPR actually began.
First 12-month TPR- Provider Notice (Qp2d):  
  • Ticket has been assigned and in-use for more than 36 months
  • The 1st 12-month TPR is past due (i.e., Ticket was assigned between 01/2002-07/2002, 24-month TPR was not sent and beneficiary was deemed to pass, and 1st 12-month TPR was due 1/2005-07/2005)-- (will only be mailed 8/2005-9/2005)
  • SSA’s system reflects less than 3 months of earnings at/above SGA during the 36 months that the ticket has been assigned and in-use
  • EN/SVR only needs to respond to the one question, which assesses whether or not the beneficiary is expected to work with earnings at/above SGA for 6 of 12 months, beginning with the month the 2nd 12-month TPR actually began.
First 12-month TPR- Favorable Outcome - Provider Notice (Gp2):  
  • A ticket-holder will receive a favorable outcome to the 1st 12-month TPR in one of 4 ways:
    • There is pre-existing evidence of earning (3 mo at SGA) and the EN responds favorably to the Qp2 notice, or
    • There is no pre-existing evidence of earnings or less than 3 months at SGA, but the EN responds favorably to the Qp2 notice and provides evidence of earnings sufficient to meet the review criteria, or
    • There is pre-existing evidence of earnings and the EN’s response was incomplete, but the responses provided were favorable, or
    • There is pre-existing evidence of earnings and the EN did not respond to the Qp2.
  • Only Providers will receive the favorable outcome notice Gp2.
First 12-month TPR- Unfavorable Outcome Notices:
  • A ticket-holder will receive an unfavorable outcome to the 1st 12-month TPR on one of three ways:
    • The EN responds unfavorably to the Qp2 notice
    • The EN responds favorably to the Qp2 notice, but there is no evidence (pre-existing or provided by the EN) that the ticket-holder met the work/earnings criteria
    • The EN does not respond to the Qp2 notice and there is no evidence that the ticket-holder met the work/earnings criteria
  • The notice lists the possible unfavorable outcome reasons and one or more items will be checked to indicate the specific reason(s) that particular ticket-holder.
First 12-month TPR - Provider Notice (Fp3a):  
  • In the Fp3a notice, the due date of the next 12-month review is 12 months from the date of the unfavorable outcome notice
  • Fp3a notices will only be sent for ticket-holders for whom a Qp2a or Qp2b was sent.
First 12-month TPR - Provider Notice (Fp3b):  
  • In the Fp3b notice, the due date of the next review is less than 12 months and is the actual date that the ticket-holder’s TPR should be completed (i.e., based on the ticket assignment date).
  • Fp3b notices will only be sent for ticket-holders for whom a Qp2c or Qp2d notice was sent.
Beneficiary Notices - English: (F3a) or (F3b) ; Spanish: (F3aS)  or (F3bS)
